I give permission for my child to attend and participate in Mount Moriah Church of LA's Children's Church ministry activities, including worship, lessons, and group activities.
MEDIA & PUBLICITY AUTHORIZATION AND RELEASE
Mount Moriah Church of LA may photograph, film, or record ministry activities and events in which your child participates, for use in training, promoting our ministries, and highlighting participants through print, video, our website, social media, and other church publications.
By selecting YES above, I acknowledge and agree that this authorization is given without compensation to my child or myself; that Mount Moriah Church of LA shall hold exclusive right, title, and interest, including copyright, in any resulting recordings; that the church may use them for any purpose stated above; and that I release and hold harmless Mount Moriah Church of LA and its authorized representatives from any claims arising from such use.
My signature shows that I have read and understand this form and I agree to accept its provisions.
